• Groups or individuals who have made critical contributions to the enhancement of patient care through technology are eligible to receive this award.

• Nominator’s cover letter that provides a rationale statement describing the extent to which a specific project within the nominee’s work history has had a global impact on patient care or patient safety.
• This completed Laufman-Greatbatch Award nomination form.
• A current CV/Resume for the nominee(s).
• One or more letters of recommendation written in the past year supporting this nomination from supervisors, colleagues, and industry partners.
